4. Eligibility and Geographic Limitations
Our telehealth services are available to residents of Utah and patients nationwide where telehealth is permitted. By using our Services, you represent that you are located in a state where we are authorized to provide telehealth services.
You must provide accurate and complete information during registration and maintain the accuracy of your information. Providing false information may result in termination of your account.
5. User Accounts
To access certain features of our Services, you may be required to create an account. You are responsible for maintaining the confidentiality of your account credentials and for all activities that occur under your account.
You agree to notify us immediately of any unauthorized use of your account. We are not liable for any loss or damage arising from your failure to protect your account credentials.
6. Payment Terms
Our Services are cash-pay. We do not bill insurance directly, but can provide superbills for you to submit to your insurance company for potential reimbursement.
Payment is due at the time of service unless otherwise agreed. You authorize us to charge your provided payment method for all fees associated with your use of the Services.
Prices for our Services are subject to change. We will provide notice of any price changes before they take effect.
7. Cancellation and Refund Policy
Membership can be cancelled at any time. Cancellation will be effective at the end of your current billing period.
Refunds for consultations are provided at our discretion. Medications and lab work are generally non-refundable once ordered or dispensed.

8. Prohibited Conduct
You agree not to use our Services for any unlawful purpose or in violation of these Terms. Prohibited conduct includes but is not limited to:

Sharing your account or medications with others.
Interfering with or disrupting the Services or servers.
Attempting to gain unauthorized access to any portion of the Services.
9. Intellectual Property
